To claim your SMS Insure Insurance in the app, follow the steps below:
STEP 1. Launch the GCash app
STEP 2. Tap GInsure
STEP 3. Tap SMS Insure
STEP 4. Tap Learn More under "Hassle-free claims!"
STEP 5: Tap the link under How Do I claim my insurance?
STEP 6: You will be led to the Online Service Request form. Tap Next.
STEP 7: Answer the Customer Details. Tap Next once you’re done.
STEP 8: Answer the service request details. Tap Submit once you're done.
To Claim your SMS Insure through the help center, follow the steps below:
STEP 1: Tap https://form.jotform.com/InsuranceKo/GCashInsurance_Support
STEP 2: Fill out all necessary details and attach supporting documents
The supporting documents will depend on your situation for wanting to claim your insurance. You may present necessary medical forms, death certificates etc.
STEP 3: A representative from the InsureKo team will reach out about your disbursement within the next 10 working days.
For Hospitalization claims, please prepare the following documents:
- Hospital record showing date of admission, discharge, diagnosis and treatment given.
- Government issued valid ID/s of the insured person and the beneficiary.
- If hospitalization is due to accident, violence, attempted self-destruction, police and medico-legal, reports will also be required.
Digital and/or physical documents will be identified to complete the documentation, depending on the event
